We hope this page has helped explain how to fill out the forms. They are a vital part of your job if you want to get credit for what you have done and what you will do in the future. Do Not take them lightly. The USCG relies on our submitting them in a timely fashion as they use our hours, travel, missions to show our need to the Government thus enabling then to receive money necessary to support our endeavors.
